INFORMED CONSENT AND LIABILITY WAIVER

By participating in a Face Yoga Hawaii class, I acknowledge and agree to the following:
 

Voluntary Participation
I am voluntarily participating in a Face Yoga class conducted by Face Yoga Hawaii and understand that participation involves physical movements and exercises involving the face, neck, and body.

Acknowledgment of Risks
I understand that participation may involve physical exertion and may result in physical discomfort, soreness, muscle strain, or other injury. I acknowledge that there are inherent risks associated with physical activity and voluntarily assume responsibility for those risks.

Medical Clearance
I understand that it is my responsibility to consult with my physician or other qualified healthcare professional before participating if I have any medical concerns, health conditions, injuries, or other circumstances that may affect my ability to safely participate. I confirm that I am physically able to participate in the class and will notify the instructor of any condition or limitation that may affect my participation.

 

Assumption of Risk
I knowingly and voluntarily assume all risks associated with my participation in the class, including, but not limited to, physical discomfort, soreness, muscle strains, muscle pulls, muscle tears, or other injuries or adverse effects that may result from participation.

Release and Waiver of Claims
To the fullest extent permitted by applicable law, I knowingly, voluntarily, and expressly release and waive any claims I may have against Naomi Sakai and Face Yoga Hawaii, and their respective owners, instructors, employees, agents, and representatives, arising out of or relating to my participation in the class, including claims for personal injury or property damage, except to the extent such claims cannot legally be waived.

I understand that this release applies to claims arising from the ordinary risks associated with participation in the class and, to the fullest extent permitted by applicable law, claims based on the negligence of the released parties.

Acknowledgment and Agreement
I have carefully read this Informed Consent and Liability Waiver and understand its contents. I understand that I am voluntarily giving up certain legal rights by signing this agreement. I have had the opportunity to ask questions and seek independent advice before agreeing to these terms. By participating in the class, I voluntarily agree to the terms and conditions of this waiver.
